Health Status Record
- Required annually from all DSHR Members. Members without a
current HSR should not be assigned to a DRO.
Pre-Assignment Health Questionnaire - Successful completion of
this form is required from all DSHR Members as they are assigned to
each DRO. It is required for every DRO which a member is
assigned.
Physical Capacity Grid -
Lists the physical abilities required for each Activity.
Hardship
Codes - Provides definitions of the Hardship Codes.
HSR
Review Summary Sheet - Provides a format for the Chapter Health
Reviewer to document the Restriction Codes applied to a DSHR member
to the Chapter DSHR Administrator and is placed the Member's DSHR
file.
